POSITION SUMMARY: A Certified Nurses Aide is a primary member of the nursing team who provides quality care to residents in compliance with philosophies, objectives, resident's rights and established policies and procedures.




REPORTS TO: Nurse on duty or Supervisors; annual performance evaluation by DON/ADON with input from Unit Nurses.




DUTIES & RESPONSIBILITIES:





  1. Actively participates as a "team" member showing respect, dignity,


accountability, professionalism and ethical values toward residents, family


and staff while maintaining confidentiality.





  1. Follows hierarchy and respects and performs all duties asked of them


within the standards of practice.




** 3. Meets resident ADL needs including personal hygiene, oral/denture care,


bathing, grooming, and dressing as per policies.




** 4. Follows and comprehends resident plan of care regarding physical and


emotional well-being of each resident including but not limited to all


mobility, hygiene needs, and emotional care as directed by individual plan


of care.




** 5. Has a working knowledge and demonstrates usage of Resident Rights, abuse


and neglect policy, including report requirements and protocol.




** 6. Follows all policies regarding safety measures and equipment competency,


including supply needs and replenishment.




** 7. Consistently follows all Infection Control policies and measures regarding


hand washing standard precautions, linens, clean and orderly environment


including all resident care areas.




** 8. Meets elimination needs of all residents per policies and documents


toileting times and initials incontinence and continence. Follows routine


bowel regimes, documents and reports to Nurse.




** 9. Follows Rehab Nursing measures. Some examples are encouraging


independence and decision making, range of motion, ambulating, positioning


and alignment, transfers and transport, moving resident up in bed, etc.)


Pays special attention to good body mechanics and safe lifting techniques.





  1. Is responsible to look at the activity calendar at the beginning of


each shift and assures that all residents are transferred to and from


all activity programs in a consistent, timely manner.





  1. Assists with admissions, discharges, transfers, and transport to


appointments.





  1. Reports any needs of resident, signs or symptoms of illness or distress,


injuries or accidents including falls, skin tears, bruises, and red areas


to Nurse on duty.




** 13. Measures, records and documents vital signs, height/weight, I&O's and any


information that is pertinent to resident care plan, resident care profile


and reports all out of limit findings to the Nurse on duty.





  1. Participates in resident care conferences as appropriate and the process


of care planning in a positive and constructive manner.




** 15. Follows all pertinent personnel policies re: attendance, breaks, meals,


staff meetings, mandated requirements per policy, float assignments,


dress code etc. Work schedules may be subject to change based on the


needs of the department to provide the highest quality care to our


residents.





  1. Has a working knowledge of Corporate Compliance regulations and promptly


reports any potential Corporate Compliance violations to the Corporate


Compliance Officer.





  1. Is familiar with and demonstrates Continuous Quality Improvement


principles and participates in the process as appropriate, focusing on


meeting the customer service needs of the internal and external customer.




** 18. Must possess the highest ethical standards with respect to discretion and


regard for confidential information.




** 19. Provides age appropriate care to adult and geriatric patients/residents.




** 20. Is responsible for attending all mandatory educational programs as


required by position including successful completion of orientation


program and maintains 12 CEU's per year. (6 CEU's per 6 months)





  1. In order to provide the highest quality care to our patients and


residents, individuals may be required to work hours beyond those normally


scheduled. Work schedules may be subject to change based on the needs of


the department.





  1. It is understood that this job description lists typical duties for the

EDUCATION/TRAINING: Certified as a Nurse Aide with the New York State Department of Health. Current Heartsaver certification required.




EXPERIENCE: Nursing Assistant experience preferred.




QUALITIES: Demonstrates professionalism, team effort, kindness and concern for the


residents and other employees.




EMPLOYMENT SCHEDULE: Required to work weekends, holidays and various shifts as needed.




PHYSICAL/MENTAL DEMANDS: Involves standing and walking most of the day. Required to lift, move, position and transfer/transport equipment and residents. Ability to adjust to various staff members, assignments and clinical areas.




WORKING CONDITIONS: Exposed regularly to disagreeable odors, sights, body wastes, etc.




HAZARDS: May be exposed to contagious disease and infection. May sustain injury due to


irrational resident.




EXPOSURE CATEGORY: Category I. Tasks that involve exposure to blood, body fluids and tissues.
